Extremis first appeared in Warren Ellis's 2005 Iron Man story. It is something of a counterpart to Tony Stark, but for the government to be used as a weapon.
Developed by Dr. Maya Hansen, she was concerned if it had the capacity to cure basically everything. And like most science fiction comic book action experiments, this went wrong and it turned up here and there in Iron Man stories ever since.

The sequel centers around the Mighty Avenger as he battles to save Earth and all the Nine Realms from a shadowy enemy that predates the universe itself, which turns out to be an ancient race led by the vengeful Malekith that has returned to plunge the universe back into darkness.
